I acknowledge that I have voluntarily agreed to engage and otherwise participate in the APEX SPORT FEST BJJ Cup being held by Diamondback Fighting Championship (trading as Cardio flex) at APEX SPORT FEST at Norwood Oval on Saturday 25th February 2023. I am aware that the activities constitute a potentially dangerous activity in which injury may occur. I am voluntarily participating in these activities with full knowledge of the danger involved and hereby agree to accept all risks of injury or death when so engaged.

I hereby release Diamondback Fighting Championship (trading as Cardio-flex), it’s instructors, marshals, referees, staff and other authorized agents, employees and any of its affiliated organizations from all the actions, claims and demands that I, my assignees, heirs, guardians and legal representative now have or may hereafter have for injury, damage or loss resulting from my participation in this competition at APEX SPORT FEST and its employees and all appointed staff & volunteers.

I have carefully read this agreement and fully understand its contents. I am aware that this is a release of liability and that it forms a contract between myself and Diamondback Fighting Championship (trading as Cardio-flex), it’s agents, instructors and employees and I agree to it of my own free will.
